Concierge Care Management
We understand that every client's needs are unique; therefore we offer a variety of personalized concierge services.
- Serve as the care manager for all aspects of client care
- Develop care plans
- Oversee nurses and caregivers
- Evaluate optimal living situations (in-home, facility, etc.)
- Coordinate medical appointments and hospitalizations
- Make referrals for palliative or hospice care
- Provide unplanned or emergency troubleshooting at client’s residence regarding issues such as behavioral changes or medical needs
- Coordinate Long-Term Care insurance including approvals, claims processing, and paperwork management
- Utilize network of industry experts including Physical Therapists, Occupational Therapists, Speech Therapists and Elder Law Attorneys, to provide specialized services as needed
- Arrange in-home personal services, including insurance-covered podiatry visits, manicures, pedicures and hair care
Caregiving
Our experienced team provides compassionate care while prioritizing the physical, emotional, and mental
well-being of our clients.
- Assist with Activities of Daily Living, including bathing, dressing, toileting, eating, and transferring
- Prepare and serve meals and perform light housekeeping duties
- Help with physical activity and exercises
- Provide medication reminders
- Attend medical appointments
- Communicate client’s condition to others involved in caring for the client and report any unusual incidents or behavior
- Run errands, as requested
- Engage clients through conversation and companionship
Nursing Consultation
Nursing Consultation
Our dedicated team of licensed nurses provides nursing services to ensure comfort and safety.
- Serve as in-home registered nurses, coordinating closely with our client's medical team
- Address in-home medical matters, as appropriate
- Manage medications
- Create and implement individualized care plans
- Attend and coordinate medical appointments and interactions with medical providers
- Communicate with client, family members, caregivers, care manager and others, as appropriate
